The invasive electrophysiologist must use all tools at his or her disposal, including careful analysis of the surface ECG, analysis of multielectrode fascicular signal sequence, mapping the early ventricular site (while understanding the futility of ablating at this site), mapping the early Purkinje activation, and the comparative interval measurements explained above to be safe and successful in eliminating this difficult and malignant arrhythmia.
