Abstract

Portal hypertensive gastropathy (PHG) and gastric antral vascular ectasia (GAVE) are two distinct types of gastric mucosal lesions that can result in bleeding in patients with liver disease. In most cases, the diagnosis can be made during endoscopic evaluation, although for a few cases, histology may be necessary to confirm the diagnosis. It is important to distinguish PHG from GAVE as the two processes have significantly different pathogeneses, which has led to the recognition that patients with PHG versus GAVE require different management strategies. In this chapter, we review the pathophysiology, diagnosis, management, and future directions in the management in PHG and GAVE.
