Androgen deprivation adversely impacts libido and sexual potency, psychologic well being, mood and (possibly) cognition, exercise tolerance, muscle mass, bone mineral density, intra-abdominal adiposity, and lipid and carbohydrate metabolism [32,34]. Collective data identify presumptive impairment of hypothalamic GnRH outflow, reduction in pulsatile LH-stimulated testosterone synthesis, and decreased negative feedback in older men. Understanding the primary mechanistic bases of testosterone depletion in aging men may foster novel therapeutic strategies to maintain anabolism, enhance quality of life, and minimize physical frailty.
