Abstract

In industrialized countries endometrial carcinoma (EC) is the most common malignancy of the female genital tract and the fourth most common form of cancer in women. The generally accepted management of EC includes radical hysterectomy, removal of the adnexa and an appropriate operative staging of patients when there is a risk of extra-uterine carcinoma. In this article the possible indications for an individual, disease-oriented postoperative therapy for patients with endometrial carcinoma will be discussed. Relevant themes are the routes for lymphatic spreading and operative staging of EC, the definition of various types of metastatic spreading, introduction of the concept of disease-based postoperative therapy as well as the development of guidelines for the postoperative therapy of endometrioid and endometrial carcinomas based on experience with disease-oriented therapy and retrospective studies.
