Abstract

Over the past decades the term Coronary Artery Disease (CAD) epitomized the involvement of the epicardial coronary circulation in the atherosclerotic disease process, classically in conjunction with the visualization of luminal narrowing by angiography. However, a disease state of the coronary arteries can develop independently from the systemic atherosclerotic disease process, for instance, as part of a vasculitis syndrome such as Kawasaki disease or systemic diseases such as amyloidosis. Moreover, CAD functionally involves the entire coronary circulation, including the myocardial microvasculature. This chapter will review CAD as part of the atherosclerotic cardiovascular disease (ASCVD) process. Specifically, it will focus on current concepts of its pathophysiology as it underlies clinical presentation and therapy.
